Suffering a minor heart attack a few years back, which at the time Beyond was certain was his turn to die, he got pulled out and just kept on going. After the third year locked up, the slight worry over another heart attack went away, and he just enjoyed himself, as much as one could locked up.
The security was admittedly loosened due to Kira and prisoners weren't expected to live long, human element played its role and if B wanted he could rather easily escape, however there was no need to at that point.
During the seven year Beyond didn't have any visitors, if you don't count various therapists who never came back as visitors, so when such person finally showed up it was rather a surprise for him.
